| Calling Cards for the State of Utah
are provided by Qwest Communications. The calling
card is a cost-effective, convenient tool designed
for state employees traveling on business. |

Process: |
Qwest Communications was recently
awarded the state's calling card business. What
does this mean to you?
All existing State of Utah AT&T calling
cards, approximately 1,900, will cease to work
effective July 31, 2005. Therefore, state employees
requiring a "new" or "replacement" Qwest
calling card should submit a request to their
agency telecommunications coordinator. Agency
coordinators, in turn, should e-mail requests
for calling cards to ITS' Telecomm Order Box
( ITSORDERBOX@utah.gov ). If you don't know who
your agency coordinator is, then send your request
directly to the Telecomm Order Box email address.
Include all pertinent information, e.g., specify "new" verses "replacement",
DAS ID/Low Org, user/card holder name, authorized
person, contact information and to whom the calling
card(s) are to be mailed.
To prevent any possibility of fraud, please
destroy your AT&T Calling Card.
Time is of the essence. Ideally Qwest calling
cards will be issued prior to July 31st; the
date AT&T calling cards expire.

| Dialing Procedure |
- Dial the toll free access number
on your calling card. Listen for the voice
prompt.
- Dial your 10-digit card number + PIN.
Listen for the voice prompt.
- To place a domestic
call: dial area code + telephone number. To
place an international call: dial 011 + country
code + city code + telephone number.
- To place
calls from a touch-tone phone outside the U.S.
and Canada :
- Dial the toll free primary access
number for the country you are in. listen for
the voice prompt or tone.
- Dial the security
access code (647). Listen for the voice prompt
or tone.*
- Dial your 10-digit card number
+ PIN. Listen for the voice prompt.
- Dial to
call back to the United States : dial area
code + telephone number. To call between or within
other countries: dial 011 + country codes +
city code + telephone number.
*If the call does not complete using the primary
access number, or if there is no primary access
number, please use the alternate access number.
The alternate access number does not require
the use of the security access code. |

| Dialing Tips |
- To place additional calls, do not hang up:
press and hold (#) for two seconds.
- If the telephone you are using
does not allow the (#) key to be held down
for two seconds, press (#) several times in
rapid succession. Listen for the voice prompt
and dial the next telephone number.
- If at any
time you make a mistake in dialing, press
(*) to return to the previous step.
- To speak
to an operator, dial the toll-free access
number on your calling card and either press
(0) or hold for an operator.
- To use speed
dial, enter your card number + PIN, then
dial (1) + (#) and follow the voice prompts.
- Any additional toll charges levied by the
local telephone company are the responsibility
of the caller.

| Qwest's calling cards prompts are
delivered in English, however, Qwest can provide
language translation services as required. |

or access online order form. Your card will be
delivered by U.S. Mail within five business days. |
